Important privacy reminder
Do NOT enter any patient identifiers.
No names, dates of birth, addresses, phone numbers, Medicare numbers, or real contact details.
Use generic placeholders only, e.g.
“55-year-old male with type 2 diabetes”
“woman in her 40s with COPD and obesity”
No personal or identifying information should ever be typed into the chatbot.
How to use
- Describe the patient’s chronic condition in general, de-identified terms (e.g. “type 2 diabetes, smoker, overweight”).
- You may paste de-identified health summary information into the request box (medications, conditions, pathology values, lifestyle factors, etc.).
- The Care Plan Builder will generate a clean, ready-to-paste care plan using GPTC templates.
- You can ask for revisions, additions, or more detail if needed (e.g. more goals, more lifestyle advice, tighter SMART goals).
- All care plan content comes from GPTC knowledge only.
What this tool does
- Builds a complete GP chronic condition management plan (item 965)
- Builds a Care Plan Review when you request it (item 967)
- Produces SMART-style goals
- Fills in the four-column GPMP table
- Adds review and follow-up instructions
What this tool does NOT do
- No medical advice, no fine-tuning to the individual needs of the patient
- No diagnosis
- No prescribing decisions
- No clinical judgement
- No real patient data
- No external internet research
